These programs are designed for individuals leaving a group health and dental plan, often due to a job change or loss of employment. Coverage options are available that allow you and your dependents to continue your health and dental coverage without interruption.

Applications must be submitted within 60 days of losing group coverage. Coverage is guaranteed, with no medical questions required.​
